and they will be able to answer any questions about filling out the form.

*Sign both sides of all pages
*All handlers must submit a 1x1 photo of themselves for badges
*Must submit a copy of certified birth certificate for driver
Full Membership $150 (Has voting rights for ALQMA and National)
Alternate Handler $100 (Has voting rights for ALQMA only)
Associate Member $75 (No voting rights)
